What steps are needed before admission?

The admission process is actually quite simple and our helpful and friendly staff will walk you through each step.

Your primary physician must provide:

  • A signed History & Physical report no more than 30 days old.
  • Current signed medication orders.

You must provide:

  • A completed "Application for Residency" which can be found here.
  • Copies of documentation regarding Guardianship, Power of Attorney, Living Wills, DNR, and/or other end-of-life issues if applicable..

Our staff must perform a pre-admission residency assessment.

After these steps are taken, you will meet with our staff to complete the monthly lease agreement.

What type of payment is accepted?

The Cottage of Spring Branch is a private pay residential community, therefore we do not participate in any Medicare or Medicaid programs. However, long term care insurance often pays for assisted living. Check with your insurance provider. In addition, if you are a Veteran or the widow of a Veteran you may be eligible for special assistance through the VA.

We are licensed through the state of Texas Department of Aging & Disability Services (TXDADS) for Type B Assisted Living and Certified in Alzheimer’s and Dementia Care.

What items do I bring for my family member?

Before move in we will provide you a list of necessary items that includes, but may not be limited to, seasonal clothing, small keepsakes, family photos, medications and personal medical equipment such as wheelchairs, walkers, or canes. We will provide furniture but you are welcome to bring as many personal furniture items as space permits.
